I was recently married in May of 2001,and took my husbands last name. I now want my maiden name back, but I do not want a divorce at this point. Do I need to get a court order to go through this process, or can i down load the appropriate forms on the internet at findforms.com without having to use a court order. I need my drivers licensce changed, along with my social.

Re: name change

You have two choices. First, the law allows you to use whatever name you want to use. No formal action is required. If you want to go to court and change it formally you can hire a lawyer or get the forms from the court clerk.
