Change of Venue

Can someone tell me how to fill out the paper work needed to change venue from one county to another in a contested gaurdianship case when both parties reside outside the county the case was filed in?

Re: Change of Venue

File a notice of motion requesting a change of venue. This motion must be supported by a declaration that none of the parties or the subject of the guardianship does not live in that county.
