child cusody

i want to grant custody of my unborn baby to my mother. what are the legal forms that we must fill out in order for this to happen. I am 6 months pregnant, when the baby is born i want my mom to be the legal gaurdian, i want her to take the baby home from the hospital when it's born. Im giving custody over so she won't have to pay any adoption fees will she?

Your mother should retain an attorney to assist her in this matter. All adoptions must be approved by the department of social services.
