Child custody case

I can't find the father of my child and have started a custody case. I have advertised the summons in the newspaper, but the judge now says I have to also serve him with the CRS. I haven't seen a CRS advertised anywhere, so where can I find the wording to put in my ad? Is there another way I can go about this?

file a motion to request permission to serve by publication. you will of course have to include a declaration of due dilligence. have you tried hiring a PI to locate him?
