Child Custody

I am a single mom, when my boyfriend found out I was pregnant he left me, and he has not being around much since then. He sends me a check for $500.00 and he has seen my son one time when he was 7 months. He always request to see him but always cancel at the last minute, I have all the text to prove the he cancel every single time. My son is now 14 months and he wants to change his last name to his, can he do that? I'm the only parent named in the birth certificate and passport of my son.

the name can be changed with your consent or by court order if you do not consent.
