child custody & visitation

What forms do I need to file a request for joint custody and visitation? I am a ncp and the cp refuses to let me see our child. We were never marrried and the child is a year old. I can not afford an attorney. Please help

You have to file a petition to determine paternity. Assuming that you are the father will result in your paying child support and having some visitation rights.
