Child Responsibility Waiver

Hi, what form do I use to have my girlfriend sign stating that she will be the parent of our child and solely responsible for it, never bothering me for child support or anything else, never contacting me in the future etc.

That form does not exist. Mother cannot bargain away the child's right to receive support from the father.
