Children's guardianship in event of parent's death

What kind of form do my husband and I need to fill out and have to protect our children from going to the state if we were to die unexpectedly? We want my parent's to have custody if something should happen to us and want to prevent even temprary foster care. I was told if I didn't have anything written down, my children would go into foster care until the state decided where they would be placed. Thank You -

you can name a proposed guardian in a will or a trust.
