What are the consequences of violating/not following a stipulated agreement that is part of Court documents (i.e., refusing to participate in co-parent counseling)? Can one be held in contempt of court?

Yes they can be held in contempt. Unless it is severe case of violation usually something trivial or occasional is not sufficient for contempt and the judges have a way of dismissing cases.
