Contempt in California Family Court

How do I find out how to file a contempt action? I have used more than an years' income on atty's and do not have any more money to hire one.

When I tried to get help from the court facilitators office they said contempt was too hard for a pro per, that I would be held to the same standards as the district attorney and would fail.

My Ex is in violation of orders regarding the children, also hasn't followed three orders to produce income documents regarding hidden income, hasn't completed court ordered drug counseling per order.

Is there somewhere on line I can see a sample or get direction?

There is a judicial counsel form for filing an OSC for contempt.
