contempt for child visitation order violation
I am filing contempt charges against my daughter's mother for violating my court ordered visitation and legal custody orders. On the order to show cause and affidavit for contempt (form 410) and affidavit of facts constituting contempt (form412) Family Law, I have to list each count seperately. I listed 4 counts. 1)Violation of Visitation orders for not allowing me visitation as ordered. 2)Violation of Visitation order by moving and not informing me of the new address, phone number. she moved in 2004 and I do not know where she is. She knows how to reach me. 3) violation of Legal joint custody order. by moving and not informing me of address or telephone #. 4)Legal joint custody order. Order states I am to be notified and allowed to participate in all important decisions regarding school and medical. I have not been allowed to even know what school or doctor she goes to. Would I attach the counts to the order to show cause 8(d) FL form410 or to affidavit of facts FL412? What specific information do the counts have to contain? 8(d) on FL410 what information should I put here? I filed these but have to redo them. The counts were not specific enough. So what do they want. I can E-Mail a copy of the paper work I have.

break down each count specifically. the courts want to know how many times she violated specific orders. they want dates and times. you can call my office for a 1 hour consultation. it will only cost $275. we should be able to sort it out during that time.
