Legal Question in Family Law in California

I and my ex have been in court fighting about custody, we have come to an agreement. I just wanted to know what for I need to fill out to submit to her attorney. I fired mine due lack of response.

If your ex has an attorney, that attorney should prepare the paperwork. You will bring it to your attorney or hire an attorney to consult, that is to review your ex's proposed written agreement. If ok, you can sign off as yourself in pro per.
