custody

i am in a custody battle for my 7 month old son. someone has falsly accused me in a declaration . i don't have a lawyer but i need to subpoena her to court so that i can clear my name .what form do i use? her address is unknown but i have a telephone #

You are risking the custody of your son on your ability to represent yourself in court. Anyone who represents themselves in court is taking a big risk.

A subpoena is issued by the court clerk, but it will be your job to get it served. A private investigator is in the best position to find this person. You could also make a motion in court requesting either the address be provided to you or that the court order the sheriff or some other person to serve her.
