Legal Question in Family Law in California

custody modification

We have joint legal custody. Mother file a notice of motion to modify custody. She wants sole authority to make medical decions without my consent. In my declaration, can I ask the court to also consider granting me sole custody or must I stick to what she wants to modify. Do I need to file a seperate notice of motion?
