after death to appoint a person to take care of child

i am a notary, a friend of mine has a child who she shares custody with ex husband- she wants to leave a document if sometimes happens to her she wants her daughter to go to her cousin. father agrees. what kind of document she needs? is something we can download and have notarized? will that be enough?

The usual procedure is to name the proposed guardian in your will. However, if father protests he may prevail in any custody contest.
