Defamation by DCFS needs to be amended

What court forms do I need to present to the Judge to let her know that DCFS did not close the case as she instructed. I just found out that my case was still open after it suppose to be close 1 year and half ago. DCFS placed my children name in the system as them being a foster agency. My children has never been in a foster care system except one and the case is closed. My child is at home with me.
