I have done it 2 ways: (1) enclosing a form Notice and Acknowledgement of Receipt with a stamped addressed return envelope; and (2) for the uncooperative, call the prison and ask to whose attention you can mail it. In my experience, you mail it with a proof of service, the prison authorities will then serve it, sign the proof of service and return it.
