Divorce Retirement Attachment

I need to find out what form I have to file with the court to get myself attached to my husbands retirement plan through the State of CA, PERS. We are divorcing but have done the paperwork with a paralegal. I have been told in order for me to be paid my 7.5 years at his retirement, I must be attached to his plan. An attorney told me form 2337 but I am unable to find that form with the courts.
