Legal Question in Family Law in California

I divorced my wife in 2004 without using attorneys, just forms we found on line. I agreed to pay alimony and a small portion of her medical insurance. However, the documents filed did not have an end date for either of these payments. I have asked once to stop the alimony and she refused saying she needs ther income to survive. She receives disability amd medi care because of bi-polar disorder. Still, I beleive she could do something to improve her situation but has chosen not to. What options do I have to end the payments? All of our children are grown.
