How to draw a GUARDIAN assignment/letter

My sister is sending her 2 children to live with us for the coming 5 years or so. I think I might need to get her guardian assignment letter for the 2 kids in order to enroll them for school and maybe other activities. But I do'nt know how to do it. School district office told me to have public notary sign such guardian assignment letter but i don't even know how to write it up. Can anybody help to point me an example or template?

One other question is my sister's hubsband is not around, he is abroad. If the father of the 2 kids cannot sign it. the guardian assignment will only be signed by the mother. Will it be good enough? It just for school registration, tho.

You might try going to the local family court and ask the clerks for a caretaker agreement and similar forms. It will come with an explanation and info on your rights and obligations.
