Is an ex-spouse required to provide the address where a minor child resides? (thus her address, for the purpose of serving documents) If so, how may I find the code citation?

Not always. In fact in some situations, such as domestic violence cases, the other spouse may be prohibited from even trying to find out where the victim and child(ren) reside(s). So it depends on the specifics of your current orders.
