family name change

my husband and i got married two years ago in the state of nevada. we are california residents. i would now like to change my last name and take on his. what form do i fill out...one for california or one for nevada?

also i would like to change my 9-year-old son's last name. do i need legal permission from his biological father to do this?

You can change your name in California. You will need the bio dad's permission to change his son's name, unless his parental rights have been revoked.
