What forms do I use to file for other parent to provide life insurance? Other parent owes over $120,000.00 in arrears for child support in California. Thank you, in advance for your expertise!

There is no form for requiring that he maintain a life insurance policy. Even if the court were to order him to pay the premiums, it is unlikely that he would pay. You should consider obtaining a life insurance policy on him and pay the premiums.

You could file a motion with the court requesting that he provide security for the payment of child support. If he owns property the court could order that it be used as security. Also record a child support judgment with the county recorder where he lives or you think he may own real property. If the proper document is recorded there will be a lien on any real property in his name. Conslult with an attorney to determine what document you need to record.
