how do i file contempt of court?

hi there, my boyfriend is working right now so im gonna ask his question for him. he recently got a divorce and in the divorce agreement it is stated that she (the ex wife) is to revert to her maiden name and not use his last name anymore once the divorce was final in september. well, she has been using it and even named her bastard baby with that last name. so she is now in contempt of court. what forms or paperwork does he have to file against her?

You may request that your name be changed back to your maiden name but you don't have to. If you do change it the court doesn't care if you continue to use your married name as long as it isn't part of some sort of fraudulent plan. I believe that she and her baby's father (babies are NEVER called bastards anymore) may pick any first and last name that they want to give the baby.
