how to file to have parental rights of the father relinqhuished?

i live in california and my ex husband lives in nj(somewhere) he has not had contact with his children in any form for 9 months. is it true that if 12 consecutive months pass without any form of contact with his children, he looses his parental rights? if not, how do i file to have him served? what forms do i file? do i need an attorney to do this? basically, what is the process to follow to request that he relinquish his parental rights?

It is unclear what you mean. He loses no rights automatically. Any change in custody or visitation is made by the filing of a motion to modify the court order and you must provide written declarations showing good cause. an attorney is not required.
