I have filed foe divorce my soon to be ex husband is not helping much can I move back home soy family can help me I have two small kids. Is there a form I need to fill out

The answer largely depends on where you intend to move. If you wish to move the children out of state or a significant distance ,you should get court orders permitting this relocation or run the risk that the other parent gets orders mandating the children's return.
