how to fill out a prenuptial agreement

I am filling out a premarital agreement form I downloaded from findlegalforms.com. I am not sure what several ''closing parentheses'' symbols mean; if they're typos or if they're meant to be there. The symbols appear after the spaces left in the form for the name of the state, and of the county. Also, there is the following sequence of symbols which I do not undersand= )ss: Please let me know what to make of this. Should I cut out the symbols? Should I leave them there? Do I need to fill out extra information? I thank you in advance for your prompt response.
