Legal Question in Family Law in California

When filling out court papers for modification of Child Visitation/Custody. Can I use form #FL341-D in the request for modification? I'm also using form FL-311. But the form FL-341-D offers more choices so my question is can I use both of them together? Thank you for your time.

Talk to the Family Law Facilitator or Self-Help office at your Family Court. They will be far better able to advise you on what forms are accepted in your local court for what purposes.
