Legal Question in Family Law in California

I need to find out how i can drop a parenting plan my ex continues to violate. before this plan i filed an r.o against him and it included custody ( giving me full custody) and him limited visitations. I got the full three year r.o but judge ordered me to close my custody case with our signed parenting plan. He never see's the kids like he agreed upon and has moved numerous times without notice once out of state. The times my children have been with him he neglects them and leaves them with babysitters whom i dont even know. I have to force my son to go with him when he does not want to. I just want this plan dropped somehow so me and the kids can continue our stable lives and he can see them whenever he comes back to the county like we have been doing. "note" in the plan it states that if he moves more than 30 miles away or out of county this parenting plan is null and void but he thinks its still in effect. what do i do???

File an order to show cause why the custody and parenting plan should not be modified to what you think it should be. You have to have a court proceeding to change it.
