Fl 301

I filed an FL-301 along with all other informaton, have a court date set, I forgot to Check two boxes, how or what do I file to fix this?

Prepare a new form checking all of the boxes that should have been checked. Then change the title to state Amended at the begining the title of the document. Then file the amended document. Do this as soon as possible.
