What form do I need in California to regain guardianship of my daughter?

You need to terminate the guardianship of your daughter. You have to file a Petition to Terminate Guardianship, Judicial Counsel form GC 255. Google Petition to terminate Guardianship and go to the California court's self help website. They have a list of the forms you will need to file.
