What is the form number for a Supplemental Responsive Declaration? Is it the same form as a Responsive Declaration? FL-320?

There is none. If there is something additional that needs to be addressed, either file it on "pleading paper" (lined 1-28 in the margin) or use Declaration MC-030 from the Judicial Counsel Forms. Keep in mind this is not usually done, so be sure it's necessary to address a shortcoming in what has already been filed with the court and do your best to get it filed a few days before the hearing -- don't annoy your judicial officer.
