Here is a link to the form you need. You will need Adobe Acrobat to open the file. If you don't have this program, you can download it for free from the Adobe website. You can fill the form out online or download it to your computer. More steps will be required (e.g., serving the form on all partys and filing it with the court) to complete the substitution. It would be best to retain a new attorney first to advise you.
