What forms do I need to file?

I currently live in California. I have lived in West Virginia, where my family is. My child was born in California. I currently have an order of custody and supprot. I am wanting to leave the state to go back to West Virginia where my family is. Also, the non-custodial parent is not current on support, he hasn't taken some classes that he was suppose to 5 years ago, and his visitation is not stable. What forms do I need to file to be able to move and enforce child support?
