What is the name of the form used when a spouse wants to give all financial responsibity of a property to the other spouse?

Also, who has to sign the forms in order for them to be legal?

I am not sure that I understand your question, but it appears you want to make the other spouse responsible for the payment of community debts. A court order is normally the procedure used. However, there is no document that he can sign or order the judge can make that will relieve from the contractual duty to pay a debt. That is if your name is on the loan you will be responsible to pay the loan. The only way to be totally relieved from a debt is to get the creditor to agree, which is vertually never done. Therefor he would have to refinance the loans or switch the credit card bills to a card that he obtained after the date of separation.

If you are referring to a transfer of real estate property, then the most simplistic form of transfer would be execution a quit claim deed. Like with all contractual agreements, the party to be charged with performance is the main person needing to sign the document(s) at issue.
