What forms do I use

What forms do I use to gain emergency custody of a child. I have enough on where she has violated our court order numerous times in differnt areas of it and she has since cancelled all visitation with no cause. She has also moved out of state without any word to me or the court.?

Contact the local district attorney's office and report her taking the child and denying you visitation. If you have a certified copy of a court order giving you visitation they will assist you in finding her if necessary, and gietting visitation.
