forms needed to apply for reinstating spousal support

I have a friend who needs to have her spousal support reinstated. while she was in prison her ex had her support stopped but the judge allowed her to reapply after she got out. what forms does she need to file since she has no money and no lawyer will take her case on cosignment?

She needs to file a motion with supporting declaration and financial information. She may be able to get the court to waive the filing fee, because of her financial status.
