giving up rights?

My ex girlfriend wants to give up rights to our 2 month old daughter. I dont mind having sole custody my only concern is what paper work i need done. She says getting a paper saying she gives it to me will do with withness...is that true? what must i do to get it legalize, just in case she comes back later and demands the child back?

You need more than her declaration. You need a court order. You should probably meet with an attorny in your area to set that up.

You need to file a paternity action and serve that on her. You can then enter a stipulated judgment. The facilitator's offce might be able to help you with that, however, if you want a parental rights termination that will hold up to later attack, which is more likely than you would care to believe, I would suggest that you have an attorney prepare the agreement and judgment.
