Guardianship

I went to court & got guardianship of my gr-granddaughter.The judge said the mother lost all rights to her, she didn't even give her visitation rights.But my problem is it wasn't put down on paper.Now I'm having problems with the school saying that her mother could come & get her anytime she wants to & I can't do anything about it.I have the court papers signed by the judge stating I have guardianship of her but not that her mother lost all rights to her.Is there anything I can do to get this straighten out?Mom supposed to go into a drug rehab program ordered by court.I'm afraid if she goes with her mom & she gets caught with drugs I'll lose that little girl.Is there anyway I can get a restraining order & how hard is it to get? Sorry about so many questions but I'm trying to keep that little girl from being hurt anymore then she has already been. Thank you and God Bless
