Guardianship form questions

Yes I am looking for some help on which forms to fill out for the guardianship of a minor child, In the event that the custodial parent has been incarserated,or has deceased. The custodial parent has all rights to the child and wants to name a guardian for the child if by some chance that the parent is no longer capable of providing care of the child can you please advise me of the right forms that I need?

This is a complex issue and you need to consult with an attorney. Generally a parent appoints a guardian in their will.
