Guardianship-divorce style

Paternal grandfather is divorcing 2nd wife (non-blood related grandmother of child); stipulation has been signed by child's parents (divorced) giving guardianship to both grandmother/grandfather; Grandfather wants custody, grandmother currently has (non court awarded) physical custody of the child... (Child resides with Grandmother). How does grandfather maintain guardianship?
