Legal Question in Family Law in California

I hired a paralegal in 2001 to take care of my divorce. I received a judgement form 1287 from the court that "Judgement of dissolution be entered. Marital status is terminated and the parties are restored to the status of unmarried persons - on the following date (1-28-2002)"

Great. I just heard from my Ex last night that he was contacted by the courts that this case will be removed in February as incomplete. I called and they said the paralegal never completed her job and did not file correctly leaving me married to my ex-husband.

I printed out forms FL-170, 180, and 190 and need to fill them out and submit to the courthouse with 2 self-addressed envelopes for myself and my ex. We will then receive documentation that the divorce is complete.

My question is do I fill out the forms exactly as they were filled out in 2001 under my old legal name or do I fill them out under my new legal name?

Old name. Also be sure to mark the box in the judgment that it is "nunc pro tunc" to 1/28/2002.
