It is extremely difficult to have a parent granted no visitation. The court would opt for supervised visitation before that. If he has violated an order, you should consult an attorney regarding how to proceed next. This would depend on the degree of the violation and the nature of it. If it is severe enough, the court or the police will intervene. If not, they will not do anything to encourage the fighting, because the fighting is very emotionally harmful for the child.
If the only violation is not making the visits as scheduled, you won't get anywhere with contempt, but you can probably have the visitation time cut. You would file an Order to Show Cause on a Judcial Council form, asking for an order to show cause why visitation should not be reduced. You can get help with that from the Family Law Facilitator in your county's Superior Court.
