My lawyer dropped out of my case, due to personal reasons he moved out of the county and did not provide his new address. The hearing is within 10 days and I could locate him
to sign the Substitution of Attorney so I can take over my case. Is is possible to
file a Substitution of Attorney without his signature?

Did he move out of the country or the county? Not sure if you have a typo. You also state that you could locate him. Not sure if that is supposed to be "could not." The attorney will need to sign the substitution of attorney, otherwise you will have to file a motion with the court.
Make sure you go to your hearing and tell the Judge what happened with your attorney and you will be able to represent yourself, or hire another attorney for the hearing.
Mr. Roach and Mr. White are both correct. If you know where the attorney is call him and find out what happened and fax him the substitution of attorney. If you can't find him follow what Mr. White told you to do. However, start looking for a new attorney.
