Legal Custodial Agreement?

My ex-girlfriend and I went to court re: custody for our daughter. My ex was supposed to have the document drawn up and returned to the court for the judge to sign. Since that time we have made modifications to the agreement which she was again supposed to have the judge sign. My ex has recently denied me my visitation rights, and I was told that the police could not enforce the court order unless I have a copy of it. I do not have a copy of the document, and when I went to the court to get copies of the documents, neither were in the case file. Are either of these documents legal without a judge's signature? Is it possible that a legal document exists but wasn't placed in the court file? What are my options?

write up the order yourself and have the judge sign it.
