Legal Forms Available

I was adopted in the State of California in 1961. I understand that the only way to obtain a copy of my original birth cerficate with my birth parents information is to have a judge "UNSEAL" my records. Is there a generic form that I can complete that a judge will sign off on? And if so, how may obtain such a form?

Please advise.

The statute only allows the unsealing for a showing of necessity; it is not automatic.

Some courts do have a form: I have seen one. But I do not think there is a "generic" form

for all the counties.

You will need to show significant reason, above curiousity, to get the record unsealed.

There may be other ways to get the information you want. Sometimes you can get access to the

judgment roll, which may contain the decree of adoption, which in turn may provide information. There

are also many registered adoption searchers, whose specialty is helping adoptees find out about their

biological relatives.
