Legal Ward name change with Social Security to match all other juvenile records

I am the legal guardian of my 17 year old nephew, Cody. The courts awarded guardianship to me when he was 15. He was assigned a social security number before he was a year old under his birth father's name. His parents split before he was two. His mother always used her maiden name as Cody's last name. All of his school, court, medical, etc records are under his mother's maiden name. Only his social security number lists his birth father. He has not seen him since he was a baby, nor do we have any idea where he is. The problem is now to get a social security card with the name he has always been known by so that he can get a driver's license, job, and everything he will require as an adult.

Social Security says this a problem for the courts, not them. It seems like an awful simple problem, but I have not been able to determine just what the ''fix'' is.

File a petition for a name change with the court first. Then take the court order to the social security office to change the name. If the birth certificate already has the desired name, then simply bring the birth certificate to the social security office.
