I was married 15 years ago outside the U.S. - we never formerly filed with city hall. Is our marriage legally bound?

It depends on the law of the place you were married. With a few exceptions, such as incest or bigamy, U.S. law recognizes as valid any marriage that is valid where it was performed. So to answer your question you have to figure out if the marriage was valid and binding where it occurred.
