medical information

Yes, I would like to know what I can go about getting the medical information on my child's father. He has been proven to be the father and ordered to pay child support, but has not yet. All I want is to have him tell me or write down any and all of this family medical history so I can let my child's doctor know, so if anything is to happen to him then they the doctor will have some kind of background to go by. Could you please tell me if there is a form or something to send to him. I have called him and his mother a few times left messages to ask for it, but no return call back. Please help me. Thank You.

Medical information is confidential. You don't stand a chance of getting his medical history absent his consent of course.
