Medical/Dental/Orthodonticts/Optometric Reimbursement form

I have an ex-husband that was ordered to pay 1/2 of all uncovered expenses listed above. He was also to pay for medical/dental coverage. He hasn't in 5 years! I am a single mother supporting both children and am taking him back to court of the arrearages. On the form 212B,,,1285.61A in the columns Amount ordered, Amount paid, and amount due. Are these the amounts that he was ordered to pay, his amount that he has never paid (or the bill that I have paid all of), and amount that he owes? Help.I am working hard to support our children, and he refuses to pay even after sending him numerous copies of the bills. He refuses service and refuses to pay. Thank you. Chris

Have you contacted the da or organization which assists in obtaining child support orders and the enforcement of such orders. They will enforce the support order at no charge to you.

If you are doing it yourself list the amount of the medical and dental bills, and attach a copy to the OSC. Put the amount due to you in the column for amount due and the amount paid, if any in the appropriate column. The court will probably accpet this format. The important is for you to be able to prove that you promptly forwarded a copy of the bill to him.
